I saw these available on the knife block for Anolon Brunello 17 pc block set. Each knife had the name below slot. Would be nice in metal look name plate. This way you would know which knife you were pulling out instead of guessing.
Where to buy name of knife tags or labels to place on knife block?
Any place that does engraving should be able to help you out with this. If not go to McMaster-Carr (link below) and they have a variety of plates that they will engrave for you. Each would cost about $5 to $10.
